Patient File Transfer Form

The eye-bar file transfer program helps ensure continuity in your eye care and also ensures that we have your complete file on hand before your next eye exam.

If you would like to have your eye health history, contact lens history, or your most recent eyeglass prescription transferred to our clinic, please take the time to fill out the following form.

Once completed, please either return the form to us or send the form directly to your previous eye care specialist and they will send us the information. Typically, we can retrieve your information within 1 to 2 business days. Not to worry though, we also keep these forms in-store and can handle this at the time of your appointment, or while you shop for new eyewear.
